What is the purpose of the Demonstrated Effectiveness Questionnaire?

The purpose of the questionnaire is to demonstrate to U.S. Citizenship and Immigration Services (USCIS) that an educational program in which you are enrolled meets the “of demonstrated effectiveness” standard described in USCIS’s “Frequently Asked Questions” about eligibility for DACA (updated October 23, 2014).

Who should fill out this questionnaire?

You should fill out the “Student Information” section of the questionnaire, and you should ask your school or program administrator to fill out the “School/Educational Program Information” and “Authorization” sections.

When should I use this questionnaire?

You can submit the completed “Demonstrated Effectiveness Questionnaire” as part of your request for Deferred Action for Childhood Arrivals (DACA) under the Secretary of Homeland Security’s DACA memorandum issued on June 15, 2012.
